A confined space entry permit checklist is a final readiness review within a planned entry system. It should make the space, purpose, hazards, controls and responsible people clear. A completed sheet cannot make an unplanned entry safe, and a gas detector reading does not address every hazard in the space.
Before using the checklist
First determine the space classification and whether entry can be avoided. A competent assessment should establish the applicable entry system. OSHA 1926.1206 specifies information for construction entry permits in the United States, including hazards, entry conditions, roles, testing, communication and rescue arrangements. This article offers an original planning checklist; it is not a substitute for that standard, a full entry programme or local requirements.

Confined Space Entry Permit Checklist
| Review item | What to confirm or record |
|---|---|
| Space and purpose | Use an unambiguous space ID, exact location and specific entry task. |
| Validity and authorisation | Record the authorised period, entry supervisor and required approvals under the entry programme. |
| People | Identify entrants and attendants and confirm competence and the arrangements for changes in personnel. |
| Hazards and isolation | Review atmosphere, engulfment, mechanical, electrical, thermal and other hazards, with controlled isolation evidence. |
| Acceptable conditions | Document task-specific entry conditions and who will confirm them before entry and during the work. |
| Testing and monitoring | Record required tests, actual results, times, responsible tester and monitoring arrangements. |
| Communication and equipment | Confirm suitable communications, lighting, access, protective equipment and any ventilation required by the plan. |
| Rescue readiness | Verify the planned rescue service, availability, communication, equipment and access before authorising entry. |
A worked example
Illustrative example: the permit lists an attendant and a rescue contact, but the designated rescue team is unavailable during the planned shift. The entry supervisor holds entry and arranges a verified suitable rescue provision. Writing a telephone number on the permit would not establish readiness. The issue is closed only when the planned arrangements are available and confirmed for the task.
Follow-up and records
Brief the team on evacuation triggers and permit cancellation. If acceptable conditions are lost, apply the entry programme and withdraw as required. Never enter to attempt an improvised rescue. At completion, account for people, close or secure the space as planned and retain the cancelled permit according to applicable requirements. Record problems so the entry system can be reviewed.
Record the date, location or asset, findings, action owner, due or hold point and closure evidence. Use Yes / No / Not applicable only with a reason where needed. A blank or unexplained tick should not hide an unresolved concern.
Track outstanding actions in your daily safety officer report. Review recurring problems through your safety committee action log.
Common questions
Is a single gas reading sufficient?
The assessment and entry programme determine testing and monitoring. A single reading cannot be assumed to cover changing conditions or non-atmospheric hazards.
Can this form authorise entry?
No. Only the competent, authorised entry process and applicable permit can do that. This checklist supports the review.
