A LOTO isolation register provides traceability between a work task, the equipment and its controlled isolation record. It helps authorised people see which isolations are active and which work groups depend on them. The register is an administrative record; a spreadsheet entry cannot isolate equipment or prove that hazardous energy has been controlled.
Before using the checklist
Use the employer's equipment-specific energy-control procedures and authorised-person arrangements. OSHA 1910.147 addresses hazardous-energy control in US general industry and includes continuity during shift or personnel changes. Its scope must be checked before applying it to another sector. Keep technical isolation decisions and verification with the authorised people, and use the register to reference their controlled evidence.
Copy and adapt: Use this original checklist within your controlled site process. Add the applicable references and retain evidence for each decision.
LOTO Isolation Register
| Review item | What to confirm or record |
|---|---|
| Register reference | Give the entry a unique reference linked to the task or permit. |
| Equipment identity | Record the asset ID, location and exact boundary covered by the isolation. |
| Procedure and certificate | Reference the current equipment-specific procedure and controlled isolation certificate. |
| Energy sources | Identify the assessed types of hazardous energy through the authorised technical record. |
| Device and lock references | Record identifiers required by the employer system, including group arrangements where applicable. |
| Verification reference | Record who performed the required verification, when and where the evidence is retained. |
| Active work groups | Identify dependent permits, responsible persons and the controlled handover arrangements. |
| Status and release | Record active, suspended or released status, with the authorised decision and date traceable. |
A worked example
Illustrative example: maintenance on a pump is complete, but another team still has work dependent on the same isolation. The register shows both linked permits. The isolation remains controlled until the authorised release process confirms that all affected work and people are accounted for. Closing one permit must not silently change the isolation status for the second team.
Follow-up and records
Copy the field names into a controlled register and add columns for action owner, review date and outstanding issue. Avoid an unexplained green cell or the word safe as the entire status. Record the actual stage and evidence. Reconcile the register with the controlled isolation system during handover. Never use it to justify bypassing a personal lock or removing someone else's device outside the specific authorised procedure.
Record the date, location or asset, findings, action owner, due or hold point and closure evidence. Use Yes / No / Not applicable only with a reason where needed. A blank or unexplained tick should not hide an unresolved concern.
Track outstanding actions in your daily safety officer report. Review recurring problems through your safety committee action log.
Common questions
Is this a step-by-step isolation procedure?
No. Equipment-specific procedures, competence and verification are essential; the register only supports traceability.
What should happen when the register and field identification disagree?
Stop the affected work and resolve the discrepancy with the authorised isolation authority before proceeding.
